AP;'O ED AS TO FORM: tifi eider, City Attorney Budget Amendments F 1 2015 Exhibit A Increase: Equipment Replacement Fund - Motor Vehicles 26,182 Explanation: A 2012 Ford F250 Extended Cab was involved in an accident and was deemed a total loss. The at fault party's insurance company has agreed to pay the City $22,827.35 for this loss. This vehicle is listed on the Vehicle and Equipment Replacement Schedule, and $4,200.00 in lease payments have already been set aside for its replacement. With the insurance proceeds to be received and the $4,200 of "lease payments" already received by the Equipment Replacement Fund from the Water Fund there are $27,027 of actual monies available to make the purchase. A budget amendment is needed to place the purchase in the FY 14- 15 Budget. The insurance company will retain the salvage vehicle.
