AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF HUNTSVILLE, TEXAS,
AMENDING THE EMPLOYEE MEDICAL BENEFIT PLAN, STARTING EMPLOYEE
PARTICIPATION THE FIRST DAY OF THE MONTH FOLLOWING EMPLOYMENT,
ALTERING INTENSIVE CARE COVERAGE, COVERING NEWBORN HOSPITAL CARE,
REQUIRING CERTAIN DEPENDENT COVERAGE TO INCLUDE BOTH MEDICAL AND
DENTAL COVERAGE, AND MAKING OTHER PROVISIONS RELATED THERETO.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF HUNTSVILLE, TEXAS, that:
Section 1: The Employee Medical Benefit Plan for the City of Huntsville,
Texas, is amended by changing Section 3.01 Commencement of Participation, to
read:
3.01 Commencement of Participation
A. Each employee in active service will be eligible to participate in
the Plan. The employee will become a participant on the later of
the effective date of this plan or the first day of the month
following the date he becomes eligible to participate in the Plan.
In order to be eligible, the employee must complete the enrollment
requirements in Section 3.02.
B. Each employee confined in the Hospital or otherwise disabled on
the date the employee's coverage would commence will be covered
under this Plan on the date he returns to active employment.
C. If, however, the employee is covered as primary under another
group plan as of the employee's eligibility date, the employee
will not be eligible for coverage under this Plan. If the
employee subsequently ceases to be covered as primary under any
other group plan, the employee will immediately become eligible to
join this Plan and must do so within thirty (30) days of the date
the employee ceases to be covered under any other group plan.
Section 2: The Employee Medical Benefit Plan for the City of Huntsville,
Texas, is amended by changing Section 11.01 A.1.A.(8) to read:
11.01 A. 1. a.
(8) intensive care room and board.
Section 3: The Employee Medical Benefit Plan for the City of Huntsville,
Texas, is amended by changing 11.02 H. to read:
11.02
H. NEWBORN DEPENDENTS
Hospital- Confined: Surgical and Birthing Centers, hospital and doctor's
charges for a newborn, "well baby" care shall be covered expense for up
to 5 days of confinement.
Non - Hospital- Confined: Routine examination and check -up charges shall
not be eligible expenses under the base or major medical sections.
Doctor visits or treatment for sickness or injury shall be treated as
any other illness.
Section 4: The Employee Medical Benefit Plan for the City of Huntsville,
Texas, is amended by changing §1.a. of Schedule A to read:
1. Employee Eligibility. The eligibility requirements for an employee are
that the employee must:
a. The employee is eligible for coverage the first day the month
after the employee starts to work for the City;
Section 5: The Employee Medical Benefit Plan for the City of Huntsville,
Texas, is amended by changing Schedule B to read:
Employee Contribution Requirements
To be covered under this Plan, each employee shall contribute the
following amounts each period for himself and his dependents:
Coverage Amount of Periodic Contributions
Employee only: -0-
Employee plus Dependents:
for medical /dental $160.00
for dental only (only for those employees
with dental only prior to July 1, 1988) $ 10.20
The employee contributions shall be made periodically and continually on
the following basis:
0% of the employee costs and
100% of the dependent costs
Section 6: This ordinance shall take effect July 1, 1988; provided, however,
that any employee taking dependent dental coverage only may continue such
separate coverage at the rates provided for in this ordinance.
