A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF HUNTSVILLE, TEXAS, INCREASING DENTAL
BENEFITS, ESTABLISHING TWO DENTAL PLANS, INCREASING DENTAL PREMIUMS, IMPLEMENTING A
WELLNESS PROGRAM TO INCLUDE PARTICIPATION REBATES, AND ESTABLISHING A TOBACCO
CESSATION PROGRAM AND ASSOCIATED SURCHARGE AS APPROPRIATE; AND ESTABLISHING AN
EFFECTIVE DATE FOR THESE CHANGES AND ADDITIONS

I WHEREAS the City of Huntsville's Employee Policies and Procedures Manual states, "The City of Huntsville
provides health and dental insurance protection to eligible employees. The City Manager and
Human Resources will be responsible for advising the City Council on the program and necessary
in-house administration";and
WHEREAS it has been determined that these benefit enhancements will assist in improving the overall
health of our employee population while having the potential to reduce claims costs.
NOW,THEREFORE,be it resolved by the City Council of the City of Huntsville,Texas,that:
SECTION 1: The above findings of fact are adopted by the City Council.
SECTION 2: The dental plan design will be amended to increase the annual maximum covered from$1000 to
$1500; and will also be amended to increase the lifetime maximum coverage for orthodontics
from $1000 to $1500, as well as establishing two options; the following premiums will be
established:
Without Ortho With Ortho
Single Employee $8.67/mo. $19.95/mo.
Employee+Spouse $15.31/mo. $29.98/mo.
Employee+Children $12.89/mo. $26.67/mo.
Employee+Family $21.36/mo. $38.14/mo.
SECTION 3: Employees and spouses on the health plan participating in a biometric screening and/or an on-
line health risk assessment will receive a rebate of$20 for each activity completed.
SECTION 4: Effective January 1, 2015, tobacco cessation classes will be offered to assist employees with
quitting tobacco use. Effective January 1, 2016, employees who use tobacco products will be
charged an additional$50 per month(tobacco use surcharge).
SECTION 5: These changes are to be implemented on January 1, 2015, unless otherwise stated. City staff is
hereby directed to take all necessary steps to implement the changes and/or programs in Sections
2,3,and 4. These changes will apply to retirees on the City's health plan.
